Dear Parents and Guardians,
The US Food and Drug Administration expanded the use of the Pfizer COVID-19 vaccine for individuals 12 years of age and older.
We are pleased to share that our school district is partnering with Bon Secours Community Hospital and Westchester Medical Health Network to offer the COVID-19 vaccine at Port Jervis Middle School and Port Jervis High School for age-eligible students with their parents’ consent.
The Pfizer vaccine is administered in two doses, three weeks apart. Below are a few important details:
- The first dose of the vaccine will be administered on June 1, 2021.
- The second dose of the vaccine will be June 22, 2021.
- Both distributions will take place in the schools, during the school day.
- Students must be 12 years of age or older.
- Registration must be completed using the following survey link. Survey Link
- The NYS Department of Health Vaccine Consent Form must be completed and turned in prior to receiving the vaccine. NYS DOH Consent Form
Other important details:
- Parental consent is REQUIRED.
- Vaccines will be distributed by Bon Secours Community Hospital staff.
- There is no cost for the vaccine.
- Paper copies of the survey and Consent Form are also available at PJMS and PJHS Main offices.
