The Orange County Department of Health will be conducting a COVID-19 Vaccine Distribution on September 27, 2021 from 2:45 pm - 5:30 pm at Port Jervis High School (Tri-States Gym.) To schedule an appointment for the vaccine, please use the following link:

The vaccine that will be distributed on this date is the Pfizer version, which is FDA approved for individuals 12 years of age and older. Vaccines will not be administered to individuals under the age of 18 years of age without written and verbal consent from a parent/guardian. The written consent must be received by the Main Office of your child’s school no later than Friday, September 24, 2021. To access the consent form, please use the following link:
