Food Service - Remote/Hybrid Model Delivery & Pick Up Request (starting 9/9/2020)

Please complete the form to receive breakfast and lunch delivery/pick up for children in your household during the 2020-21 school year while we are in a remote/hybrid model. Food will be available for pickup for students that do not require delivery services at the 209 complex, HBE, and Middle School on Monday, Wednesday, and Friday from 10:30 am - 12:00 pm. Each delivery will provide meals for the whole school week. Any child enrolled in the Port Jervis City School District will be eligible for either option. The delivery service is to accommodate students in this time of Remote/Hybrid Learning.

Meal Distribution Model:

  • Monday delivery: Lunch (for Monday and Tuesday); Breakfast (for Tuesday and Wednesday)
  • Wednesday delivery: Lunch (for Wednesday and Thursday); Breakfast (for Thursday and Friday)
  • Friday delivery: Lunch (for Friday); Breakfast (for Monday).


Please be aware that someone is required to be home at the time of delivery in order to receive the food items. The estimated time of delivery will be between 10:30 am and 12:30 pm.
