Dear Parent/Guardian:
New York State Public Health law requires children born on or after 1/1/94, to have a booster vaccination for diphtheria, tetanus, and acellular pertussis (Tdap) upon entering the 6th grade.
The required immunization can be obtained from your family healthcare provider or by the Orange County Department of Health Children’s Immunization Clinic if the child is uninsured/underinsured. You must call (845) 291-2330 to set up an appointment. Please bring a copy of your child’s immunization record with you.
Failure to provide written proof from your doctor of the above vaccination or proof of a future appointment before the first day of school will result in your child being excluded from school at the start of the 2022-2023 school year. Please send a copy of your child’s immunization record as soon as the vaccine is administered to the Health Office.
