End of the Year Medication Letter

Dear Parent/Guardian:

The end of the school year is quickly approaching! This is a reminder for you to pick up your child’s
medication in the Health Office prior to the last day of school. The school is not able to allow your child to
bring his/her medication home themselves; it must be picked up by a parent.

If you have any questions or concerns, please call the Health Office. We will be available for medication
pick up until 11am Friday, June 24, 2022. If you need to make special arrangements to pick up your child’s medication, please call us prior to the aforementioned date.

It is not appropriate for us to keep medication at school over the summer- and this includes inhalers,
epi-pen, and glucagon - so please make arrangements to pick your child’s medication up. Many of these
medications are expensive, and we are to dispose of any medication that is not picked up by the date indicated. (Additionally, if your child has a nebulizer machine in the Health Office.....that needs to be taken home as well.)

For your convenience in preparing for next year, a Medication Form is enclosed. Please have it
completed by your child’s healthcare provider over the summer if you anticipate your child will be taking
medicine during school hours in the 21-‘22 school year. Please note that your signature as well as the Dr.’s signature is required on the form. Medication must be brought to the school by a parent or guardian and not by the student. New medication permission is required for each medication each school year.

If your child has a self carry/self administer medication order, please be sure that your child’s Dr.
indicates this on the Medication Form.

All medication left at school will be disposed of as of June 24, 2022, at 11am.
