The Port Jervis City School District requires resident families to register for private schools

Registration Requirements:

• Proof of birth
• Two proofs of residency
• Parent/Guardian Photo ID
• Custody papers, if applicable

**In addition, If your child requires transportation, please complete the application provided by the private school**
