Student Registration
Welcome to the Port Jervis City School
District
This website
contains information of interest to families who
are relocating to Port Jervis,
Deerpark, or one of
the other communities served by the
Port Jervis City School District.
To register in the district, a child must have his/her
actual and only residence in the Port Jervis City School
District. The child must be 5 years of age or older before
December 1 of the current school year.
Temporary Location: Beginning June 28, the Registration Office is
temporarily located
at the High School in Modular #2.

Registration Hours:
Monday to Friday, 8 a.m. to 4 p.m. Closed for lunch 1 to 2 p.m.
Children who will turn 5-years old on
or before Dec. 1, 2008, are eligible to enter kindergarten in
September 2008.
REQUIRED DOCUMENTS
The following documents must be presented at registration:

Student's original birth certificate

One of the following proofs
of guardianship:
___Birth Certificate with registering parent’s name
___Court Judgment of Custody (See Below*)
___DSS299 for Foster Parents (See Below*)

Record of student's immunizations
-
Student's Social Security Card
-
Parent/legal guardian will
need two proofs of residency:
1. Required - Current utility bill
(O&R, cable, oil/propane, phone to residence)
2. And, one of the following:
Lease, Deed or Mortgage Document,
New York State Driver's License
*Additional documents are
required for guardians of children not living with their natural
parents and children in Foster Care. Please call Central
Registration at 858-3181 for more information.
